The Hamilton Practice - GP Registration Events for Osler House Patients

If you live within The Hamilton Practice area you can register with the practice at an open morning specifically for this purpose at The Hamilton Practice, Keats House, Bush Fair on SATURDAY 3rd MARCH 2018 BETWEEN 9AM – 12NOON

We will also be able to register patients at the Patient Information Events arranged by NHS West Essex Clinical Commissioning Group
on Tuesday 13th March and Monday 19th March (see full details in your Osler closure notification letter).

Patients should download and complete a “New Patient Registration Form” from our website before attending as this will simplify the process. You should also bring along proof of ID (e.g. driving licence or passport) and address (e.g. utility bill or bank statement).

Owing to the extremely large number of enquiries The Hamilton Practice are currently receiving we would respectfully request that patients do not contact The Hamilton Practice or attend in person in order to register other than on the dates shown above.
