My Care Record will help deliver better co-ordinated care

People in west Essex will benefit from better, joined-up care thanks to a new programme which gives health and care professionals the ability to view medical records during treatment.

Health and care professionals need to access information in order to make the best decisions about a patient’s diagnosis and treatment. My Care Record will help this to happen more quickly and accurately. Before viewing the record, patients will be asked for their permission by the professional treating them.

Dr Christine Moss, Medical Director of West Essex Clinical Commissioning Group (CCG) said: “During the My health, My future, My say campaign, the public told us they wanted to see better joined-up care. Since then, we have taken great strides in bringing together health and care services in order to improve the experience of patients.

“Our next step is to ensure that health and care professionals directly involved in a person’s care have access to the most up-to-date information about them. This will help ensure professionals have more time to spend on clinical care and reduce the repetition and paperwork“

My Care Record does not share information, but provides health and care professionals, with permission, access to view information. An example could include allowing a hospital doctor to see a person’s previous medical treatment or conditions in order to treat them appropriately.
